- Life member
- Senior member
- 5.4 status (PDF application for members who experience a hardship paying dues and/or special assessments by reason of physical disability, illness, etc.)
- Non-practicing member (PDF application for members who are in good standing with their component society and have not practiced in at least a year)
- Permanently disabled (PDF application for members who experience dues hardship from total disability that prevents them from working)
- Transfer (complete this application to initiate the process of changing component organizations, such as after a move)
