Is Your ABN Form Expired?

Advanced Beneficiary Notice (ABN) forms are available at no charge from CMS, and they have expiration dates. CMS has issued a new ABN form that is effective immediately. The previous ABN form had an expiration date of June 30, 2023. Check the bottom right corner of the form you are using to ensure it has not expired. The current forms can be downloaded from the CMS Fee-For-Service ABN page by clicking on “ABN Forms English and Spanish.” The current ABN form expires on January 31, 2026.
